CareSphere provides services through the following programs:
Act 150 Waiver
Program enables individuals with physical disabilities aged 18-59 to live in their own homes and communities
OBRA Waiver
Home and community-based services to people with developmental physical disabilities to allow them to live in the community and remain as independent as possible
Community Health Choices Waiver
Home and community-based services for adults 21 or older with a physical disability who wish to receive long term services and supports in home or community settings
VA Community Care Network
VA provides supportive medical services to help chronically ill or disabled eligible Veterans of any age remain in their home. These services are referred to as home health care and are considered extended care services prescribed by and under the direction of a VA physician.
Options Program
Pennsylvania's Options Program provides aging state residents with care services in their homes, such as personal care, home health, and home delivered meals. Assistance is also provided to family caring for a loved one at home, such as adult day care, respite care, and specialized medical transportation assistance.
Trust Funds
If you have a trust fund, ask your Trust Fund adviser. You might be eligible to use that fund for home care needs
